Insurance Information

We are a preferred provider for many of the major insurance companies. We do not participate in Health Management Organizations (HMO), Dental Maintenance Organizations (DMO), or Public Assistance.  Because insurance plans vary, we will do our best to estimate your coverage, but cannot guarantee coverage due to the complexity of insurance contracts.  You are responsible for fees charged by our office, no matter what your insurance coverage may be. It is the patient's responsibility to know and understand their insurance plan type and coverage.

Payment Options

Payment is expected at the time of service. We accept checks, cash, and credit cards. We also offer flexible financing through CareCredit.

Cancellation Policy

​

We do have a policy regarding missed, cancelled, or no show appointments without 24 hours notice. Fees may be applied.

Patient Forms

​

If you are a new patient to our office please complete the online new pa​ti​ent forms prior to your appointment. For children age 13 and under use the chi​ld forms. If you haven't been seen in our office for over one year or have changes to make, please update your information online prior to your visit.  Be sure to bring your insurance information with you to your appointment.
